Back Pain

Back pain is extremely common – about eight in ten people in the UK are affected at some time in their lives. Each year, close to five million working days are lost due to back pain. Back pain is actually very common—about three in four adults will experience back pain during their lifetime, especially low back pain. Most back pain comes from sprains and strains caused by stressful movements, and with a bit of medical attention, those will get better. Low Back Pain

Low back pain means a pain or ache somewhere between the bottom of the ribs, at the back, and the top of the legs. The pain often begins suddenly, and may follow an obvious strain or injury, but may seem to come on “out of the blue”, or come on slowly. The pain may travel to or be felt elsewhere. It often goes into the buttocks, but may go further down the leg and even into the foot.

Treatment of Low Back Pain

1) Apply ice for 15 to 20 minutes, 3 to 4 times a day or as often as once every hour. You can use an ice pack or a bag of frozen vegetables wrapped in a thin towel. After 2 to 3 days of ice, switch to heat for 15 or 20 minutes every 2 or 3 hours. You can take a hot shower or use a heating pad or hot water bottle. You may also want to try switching between heat and cold.

2) Cold and hot packs for some may help reduce pain and inflammation and allow greater mobility. Apply a cold compress to the tender spot several times a day for up to 20 minutes. After a few days of cold treatment, apply heat for brief periods to relax muscles and increase blood flow. Warm baths may also help. Patients should avoid sleeping on a heating pad, which can cause burns and lead to additional tissue damage.

Herniated disc

Many patients with back pain, leg pain, or weakness of the lower extremity muscles are diagnosed with a herniated disc. When a disc herniation occurs, the cushion that sits between the spinal vertebra is pushed outside its normal position. A herniated disc would not be a problem if it weren’t for the spinal nerves that are very close to the edge of these spinal discs. It occurs when the ligaments that support and surround the flat, plate like cushions between the vertebrae in the spine begin to weaken.

The brain’s sensory cortex, which receives and interprets incoming information, maintains a representation of the body physically within itself. The homunculus is the name given to the diagram obtained when each part of the body is plotted against its place on the sensory cortex, with more important areas of the body being illustrated as larger areas of the brain. Various areas, such as the hands and the lips, take up much more brain area due to their importance in normal function, and it is these most important areas to control which need greater sensory awareness and greater processing power to work out responses.When we suffer an injury the pain comes directly from that part, streaming in from the highly irritated nerve ends and the normally silent nerves woken up by the chemical soup of the injury. As the barrage of impulses comes in to the spinal cord it meets the second stage nerves which will take the messages on into the central nervous system. These second stage nerves become highly excited by the incoming torrents of impulses and amplify the signal significantly, passing on much higher pain levels to the higher brain centres.We don’t feel pain until it reaches the higher brain centres and intrudes upon our consciousness. In a sense, all our pain is in our minds, as it does not exist unless it gets up to our conscious brain.  Our pain is not imaginary, our brains are constantly creating a virtual reality for us to understand the world, a virtual visual reality, a virtual touch reality and also a virtual pain reality when it’s appropriate. This concept is important in that it is the brain which constructs our pain reality and not the broken ankle, the slipped disc or the burnt hand.When a limb in amputated it is obvious the muscles, ligaments and bones are all cut, but what is less clear, and much more important for the future, is that the nerves travelling down the part are also cut through. Cutting the part of the nervous system off from the centre means a sudden loss of incoming signals from the amputated part, with serious side effects for the individual. When the nervous system is deprived of its incoming information the consequences can be unpleasant.The second stage nerves react badly to being deprived of their incoming streams of impulses, not by going off-line but by doing the opposite, by increasing their reactivity and responsiveness. Because the nerve has been cut and there are not messages coming through they can begin to fire off impulses for no reason, spontaneously. These overexcited nerves can produce a significant pain problem as while the leg does not exist any more the nerves which serve the leg areas are still present in the central nervous system. The brain’s sensory areas responsible for the leg are still capable of manufacturing leg pain.Pain which appears in an area of the body which is now absent is known as phantom pain and is a common side effect of amputation which develops in the weeks and months after the trauma. Phantom pain can be very unpleasant in nature, very deep and cold, or sharp and stabbing and so can be a particularly difficult pain to treat or to cope with. Neuropathic pain is the term for a pain like this which is generated internally by the central nervous system and not as normal pains which are secondary to tissue damage.Drug treatment of phantom pain is difficult as the morphine chemicals such as morphine, fentanyl, tramadol and codeine are often not very effective. The nerve treatment agents such as amitriptyline, gabapentin and pregabalin are used against neuropathic pain with some effectiveness. Other treatments include transcutaneous electrical nerve stimulation (TENS), an electrode based stimulation treatment which can be self-managed. Cognitive therapy may also be useful to start to cope with what can be a long term problem.Phantom pain can be an intractable, serious problem for anyone with an amputation, and having significant pain before the amputation may make the likelihood of phantom pain greater. A multidisciplinary approach involving a pain clinic is most likely to be helpful.

A discectomy is a surgery done to remove a herniated disc from the spinal canal. When a disc herniation occurs, a fragment of the normal spinal disc is dislodged. This fragment may press against the spinal cord or the nerves that surround the spinal cord. This pressure causes the symptoms that are characteristic of herniated discs.

The surgical treatment of a herniated disc is to remove the fragment of spinal disc that is causing the pressure on the nerve. This procedure is called a discectomy. The traditional surgery is called an open discectomy. An open discectomy is a procedure where the surgeon uses a small incision and looks at the actual herniated disc in order to remove the disc and relieve the pressure on the nerve.

How is a discectomy performed?A discectomy is performed under general anesthesia. The procedure takes about an hour, depending on the extent of the disc herniation, the size of the patient, and other factors. A discectomy is done with the patient lying face down, and the back pointing upwards.

In order to remove the fragment of herniated disc, your surgeon will make an incision over the center of your back. Your surgeon then dissects the muscles away from the bone of your spine. The surgeon removes a small amount of bone and ligament from the back of the spine. This part of the procedure is called a laminoto

Once this bone and ligament is removed, your surgeon can see, the spinal nerves. Once the disc herniation is found, the herniated disc fragment is removed. Depending on the appearance and the condition of the remaining disc, more disc fragments may be removed in hopes of avoiding another fragment of disc from herniating in the future. Once the disc has been cleaned out from the area around the nerves, the incision is closed and a bandage is applied.

What is the recovery from a discectomy?It is not unusual for these symptoms to take several weeks to months to slowly dissipate. Pain around the incision is common, but usually well controlled with oral pain drugs. Patients often spend one night in the hospital, but are usually then discharged the following day. A lumbar corset brace may help with some symptoms of pain, but is not necessary in all cases.

The facet joints are the connections between the vertebrae of the spine. Just like the knee or elbow or any other joint in the body, they allow the spine to bend and twist. They are also stabilizing joints that prevent excessive motion and help hold the body upright. The joints are on the back of each vertebrae and they link one vertebra to the vertebrae directly above and below to form a working unit that allows movement. The surfaces of the joints are covered with a tissue called the articular cartilage. The joint itself is lined with a membrane called the synovium and the joint is enclosed in a fibrous sac, the joint capsule. Synovial fluid, a thick liquid which acts like lubricating grease and surrounds the joint, allows the bones to move without friction.When these joints become inflamed secondary to injury or arthritis, pain and stiffness occur. If the joints in the neck or cervical spine are affected, these symptoms develop:• Headaches • Pain in the neck, shoulders, and upper back.• Difficulty rotating the head• Patients often complain that they have to turn their entire body to look to the right or left.When Facet Joint Syndrome affects the lower back, there is:• Pain in the lumbar area• There may be referred pain to the buttocks and thighs• Stiffness of back• Difficulty getting out of a chair• Difficulty standing up • Complaints that the patient must walk hunched overAn injury or changes associated with aging may cause the cartilage cushion that covers the bones to wear away, resulting in pain as the bones in the joint rub together. Facet Joint Syndrome is more common in people over 50 and is usually associated with aging.Causes of Facet Joint Syndrome• Whiplash Injury can cause the syndrome in the cervical area.• Sports activities,such as gymnastics, where neck is extended• Arthritis• Poor posture which pushes the spine out of alignment• Inflammation• Infection• Degeneration of the jointDiagnosing Facet Joint Syndrome• History and Physical should be done• An X-ray, CT (computerized tomography) scan of the spine or an MRI (magnetic resonance imaging) will rule out fracture or herniated disc.• Facet joint block would determine if the joints are the source of pain.o A local anesthetic is injected into or near the nerves that supply the joint. If there is a significant decrease in pain, the diagnosis is confirmed.

Treatment

• NSAIDs (Non-steroidal Anti-inflammatory Drugs) such as ibuprophen, Naproxyn • Muscle relaxants and narcotic pain relievers might be prescribed for more severe pain.• Physical therapy • Exercise program to improve flexibility and increase pain free movement• Posture correction• Activity Modification to avoid excessive lifting, stretching, bendingApproximately 80% of the patients who follow an active rehabilitation program and take NSAIDs become pain free. This usually lasts several months.When conservative treatment fails, a surgical procedure called, Raiofrequency Rhizotomy, uses an electrical current to destroy the sensory nerves to the joint, resulting in pain relief.

There is severe pain and stiffness in your low back and hips and walking is difficult. Pain may radiate from the lower back and pelvis into the buttocks or back of the thigh. The sacroiliac joints are surrounded by a large amount of ligaments and fibrous tissue for stability. Because these joints are deep in the muscle of your buttocks, they are difficult to examine. The pain when you move may be so severe that you become unwilling to move at all and this makes the condition difficult to diagnose. Sacroiliitis can be mistaken for other causes of low back pain such as:• Sciatica• Herniated disc• Muscle strain• Septic Hip• Abscess of the psosas muscle –this is a part of the hip flexor group of muscles• Malignancy• Kidney Infection• Ankylosing spondylitis• AppendicitisCauses • Trauma, such as a car accident or fall which affects your lower back, spine, pelvis or buttocks. Torn ligaments can create inflammation or lead to infection of the sacroiliac joints.• Heavy lifting, if done incorrectly, can injure muscles and joints.• Spondyloarthropathies, which are inflammatory arthritis conditions including: ankylosing spondylitis, arthritis associated with psoriasis.• Degenerative arthritis of spine, which can cause inflammation of sacroiliac joints• Osteoarthritis leads to degeneration of the sacroiliac joints.• Pregnancy, in which the ligaments in pelvis become softer and stretch to accommodate childbirth.• Infection of the sacroiliac joint. This may be due to a bacterial infection in food such as brucellosis. This disease occurs in slaughterhouse workers, farmers, veterinarians, ranchers. It was spread through contaminated milk prior to pasteurization requirements. • Reiter’s Syndrome, which causes sacroiliac joint pain along with other symptoms.Risk FactorsBecause of the difficulty diagnosing this disorder, predisposing risk factors must be considered, including:• History of bone, joint, skin infection.• Injury to spine, pelvis or buttocks• Urinary Tract Infection may spread infection from urinary tract to sacroiliac joints.• Pregnancy, when the area around sacroiliac joints may become inflammmed.• Endocarditis, which is an infection of the lining of the heart, can spread to joints and other body organs. • Illicit IV drug use increases the risk Symptoms• Pain and stiffness in lower back, thighs, buttocks• Pain becomes worse with walking due to the motion of the hips.• Psoriasis, an inflammatory skin condition, may occur with a type of arthritis.• Pain radiating down leg, often mimics sciatica• Limp• Decreased range of motion• Elevated temperature• Bloody Diarrhea occurs with Reiter’s Syndrome which causes painful urination, joint pain, sacroiliac joint pain, and eye inflammation• Eye inflammation in one or both eyes, a symptom of Reiter’s Syndrome and often evident with sacroiliitis.It is important to seek medical evaluation promptly with sacroiliitis and to not delay treatment. This illness is no ordinary episode of back pain, and it can lead to joint destruction and the possible development of a disabling ailment or a severe systemic infection.

Discogram in which dye is injected and x-rays are taken to confirm the diagnosis that a disc is causing your pain.3. Myelogram which shows a spinal canal or spinal cord disorder after an injection of dye and an x-ray or CT scan.Obtaining an accurate diagnosis of the cause of sciatica pain is not always a simple process. Once the cause of your pain is determined, your physician will be able to develop a treatment plan that is appropriate for your condition.
